What to Expect

Welcome to My Family Dental! We are always excited to meet new patients and their families. Your first visit to our office will consist of your initial exam, x-rays, and setting up a treatment plan to achieve your desired smile. We ask for you to bring in your insurance card (if any) and a valid ID to your visit. If you are taking any medications please bring a list of their names and purposes.

Patient Forms

When you first come in you will be given your registration forms. Filling out your forms may take 10 - 15 minutes of your visit. Save some time by printing the following forms or you may also fill out your medical history form when reminded of your appointment via text message or email.

Appointment Policy

We are glad you are interested in scheduling an appointment for yourself or your children. In order to keep your teeth strong and healthy, regular visits should be every six months, which will include your exam and cleaning. It is highly important that you keep your appointment! Valuable time has been reserved for you or your child's care. A missed appointment results in lost time which could have been used for another patient.

A scheduled appointment is a commitment between you and our office, not a choice. If you are unable to keep your appointment, we require a minimum 24 hour notice so we can accommodate the dental needs of another patient. My Family Dental will charge a fee of $25 for a current patient that misses their appointment and fails to call to reschedule. If an appointment is missed more than once there will be an additional $50 added to your account for every no show. All fees will be an out of pocket expense; it cannot be billed to your insurance.
